Incoming Rollover Form
Request a direct rollover from another ABLE Plan to an Enable account. The form can also be used to deposit assets from another ABLE Plan that have already been redeemed (indirect rollover). If you do not already have an Enable account, you will also need to complete an Enrollment Form or enroll online.
Allow Additional Contributions by an Employed Account Owner Form
Instruct the Plan to allow the Account Owner to contribute additional money in excess of the Annual Contribution Limit by certifying the Account Owner is paid employee and is eligible to contribute additional funds.
